Dear new client,

When you visit me for the first time I will need to know the state of your current health in detail. Additionally I require some routine medical history, so please complete the enclosed medical background form to the best of your ability and bring it with you to your first appointment.  This will save us both a little time.  Your initial consultation will take approximately 1-1/2 to 2 hours at a cost of $120.  Credit card facilities available.

Please list any medication you are currently taking (including supplements or homoeopathic/herbal preparations).  If you have had homoeopathic treatment in the past, please list the remedies you have been prescribed, if you are able.

Panton Crescent runs off Diagonal Road (bus stop 26a), near Oaklands Road.  As you enter the street I am the 3rd house on the right (there is a ‘Dip’ sign on the left and I am directly opposite).The clinic is in the  premises at the rear, so either park directly behind my vehicle or park in the street and walk down the driveway. 

As part of your consultation, I will be taking your photo – if you have had any alterations to your facial features (i.e. teeth, nose, eyes etc) it would be helpful to provide a photo of yourself for me to peruse, showing the feature prior to the procedure.

If you need to change your appointment, please give 24 hours notice, otherwise there will be a charge.
